Medical Authorization

I, as the parent or legal guardian of the child named below, hereby authorize the staff and representatives of Immersive Beijing Learning Foundation (IBLF) to act on my behalf in securing emergency medical treatment for my child in the event that I, or my designated emergency contact, cannot be reached. I understand that reasonable efforts will be made to contact me prior to any such action. I further agree to assume full financial responsibility for any medical treatment provided to my child under such circumstances.

Activities Authorization

My child has permission to participate fully in all activities organized by Immersive Beijing Learning Foundation (IBLF). These activities may occur both on-site within the assigned facility and off-site, including but not limited to neighborhood walking tours, visits to local parks, stores, or restaurants that complement the educational program. I understand that prior notice will be provided for any off-site activities, and I consent to my child’s supervised participation in such events.

Liability Waiver and Release of Claims

I acknowledge and fully understand that participation in educational, recreational, and physical activities involves inherent risks, including but not limited to falls, contact with other participants, equipment use, environmental hazards, transportation, or other unforeseen circumstances that could result in property damage, illness, injury, permanent disability, or death.

I voluntarily assume all such risks, known and unknown, and accept full responsibility for my child’s participation. On behalf of myself, my child, and our heirs, executors, and assigns, I hereby release, discharge, and agree to indemnify and hold harmless Immersive Beijing Learning Foundation (IBLF), its officers, directors, employees, agents, volunteers, partners, contractors, and affiliated organizations from any and all liability, claims, demands, losses, or damages arising out of or related to my child’s participation in any IBLF program or activity, whether caused by the negligence of IBLF or otherwise, to the fullest extent permitted by law.

This release includes, but is not limited to, any claims for personal injury, property damage, medical expenses, or wrongful death. I understand that this release is intended to be as broad and inclusive as permitted by the laws of the State of California and that if any portion is held invalid, the remainder shall continue in full legal force and effect.

Photo, Video, and Media Consent

I grant Immersive Beijing Learning Foundation (IBLF) permission to photograph, film, or record my child during program activities for educational documentation and promotional purposes. Such media may be used by IBLF in marketing materials, including but not limited to social media posts, websites, newsletters, brochures, and news features.

I acknowledge and agree that:

• My child’s name or any personally identifying information will not be displayed, published, or shared in connection with any media.

• I will not receive compensation or require prior approval for the use of these materials.

• I release IBLF, its employees, contractors, and volunteers from any claims or liability arising from the creation, use, or distribution of such photos, videos, or recordings, provided that no identifying personal information of my child is disclosed.

Transportation Authorization and Liability Waiver

I grant permission for my child to be transported by Immersive Beijing Learning Foundation (IBLF) staff, contracted drivers, or authorized transportation providers for purposes related to the program, including but not limited to:

• Field trips to parks, museums, cultural centers, or local restaurants.

• Off-site educational or recreational activities within the local community.

• Daily or occasional shuttle services between designated locations.

Transportation may occur using IBLF-owned vans, rented vehicles, licensed charter buses, or—when necessary due to overflow, special needs, or emergencies—staff private vehicles that meet safety and insurance requirements.

I understand that all transportation will be conducted in compliance with California state safety laws and that my child will be supervised by IBLF staff at all times during transit.

I further acknowledge that participation in transportation involves inherent risks associated with travel by motor vehicle. I voluntarily assume these risks and hereby release and hold harmless IBLF, its employees, volunteers, agents, drivers, and affiliated organizations from any and all liability, claims, or damages that may arise from or in connection with transportation provided for my child, whether caused by negligence or otherwise, to the fullest extent permitted by law.
